For the freedom of choice that you're accustomed to, go with the traditional Guardian Dental insurance plan.
High Option Plan: - Includes orthodontia for dependent children under 19
- $25 deductible
- 100% coverage for all diagnostic and preventive services
- 80% coverage for all basic services except periodontics
- 50% coverage for all major services including periodontics
- 50% coverage for orthodontics (for dependent children under age 19)
- Low Option Plan
- Does not include orthodontia coverage nor major service coverage
- $50 deductible
- 100% coverage for all diagnostic and preventive services
- 50 to 80% coverage for fillings and simple extractions
Spouses and dependents up to age 19 (or to age 26 with proof of full-time enrollment) are also eligible for coverage as defined below.
Eligibility: All permanent 50% time University employees are eligible for this program.
Either you or your dental care provider may file the claims, although there is specific documentation required for certain procedures.
